LoDo Pain, LLC is a specialty chronic pain clinic providing evaluation and medication management services for adults with documented chronic pain conditions.

Medical services are delivered by licensed healthcare professionals in accordance with applicable Arizona and federal laws.

LoDo Practice Management, LLC provides administrative and operational support services to LoDo Pain, LLC.

3. No Medical Advice Through Website

Information provided on this website is for general informational purposes only and does not constitute medical advice.

Use of this website does not establish a provider–patient relationship.

4. Clinical Decision-Making

Clinical decisions — including evaluation, diagnosis, treatment planning, prescribing, and monitoring — are made by licensed healthcare providers in accordance with professional standards and applicable law.

Care is delivered within a defined Clinical Governance framework designed to promote patient safety, regulatory compliance, and responsible prescribing.

Nothing in these Terms guarantees any specific treatment, medication, or prescribing decision.

5. Self-Pay Structure

LoDo Pain operates on a self-pay basis for clinical services.

Professional services are not billed to insurance, and patients are responsible for payment in accordance with the Financial Policy.

Insurance may be used separately for prescriptions, laboratory services, imaging, or referrals when applicable.

Payment of fees does not guarantee any specific treatment or continuation of therapy.

6. Appointments & Telemedicine

Appointments are scheduled based on clinical determination and availability.

Care is primarily delivered through in-office visits.

Telemedicine may be available when clinically appropriate and permitted by applicable Arizona and federal law. Telemedicine services are subject to the same safety standards and prescribing requirements as in-person care.

Patients must be physically located in Arizona at the time of telemedicine appointments.

7. Patient Responsibilities

By using our services, you agree to:

  • Provide accurate and complete information

  • Comply with treatment agreements and monitoring requirements

  • Maintain valid payment information

  • Use communication systems appropriately

Failure to comply with administrative or payment policies may result in suspension of services or discharge consistent with applicable law and patient safety considerations.

This arbitration provision does not alter rights related to claims arising from medical care, which are governed by applicable law.

You waive the right to participate in any class action proceeding related to website use or administrative services.
