Refund Policy
Effective Date: October 1, 2025
1. Overview
LoDo Pain LLC provides transparent, self-pay pain management services.
All payments are due at the time of service. LoDo Pain does not participate with insurance, and all services are considered non-covered by insurance carriers.
2. New Patient Setup Fee
A one-time $200 setup fee is required to establish care. This fee covers your initial consultation, medical record review, and onboarding process.
The setup fee is non-refundable under all circumstances, including cancellations, missed appointments, or decisions not to proceed with care.
3. Monthly Membership Dues
Monthly membership dues are billed automatically on the 1st of each month and are non-refundable once processed.

Pro-Rated Start: Your first month will be adjusted based on your start date.

Autopay Discount: Patients enrolled in AutoPay receive a $25 monthly discount.

Late or Unpaid Dues: A $25 late fee may apply to overdue accounts. Unpaid balances may result in appointment cancellation or discharge from care.
To discontinue membership, written notice must be provided before the next billing date.
4. Refund Processing
Refunds are issued only for verified administrative or duplicate billing errors.
Approved refunds will be processed to the original payment method within 7–10 business days.
Because payment processors do not refund transaction costs, a 3% processing fee will be deducted from all refunded amounts to cover non-reversible merchant fees.
