Terms of Service
Effective Date: October 1, 2025
1. Acceptance of Terms
Welcome to Lodo Pain LLC (“Lodo Pain,” “we,” “our,” or “us”). We appreciate your trust and are committed to providing accessible, compassionate, and evidence-based pain management care. These Terms of Service (“Terms”) outline your rights and responsibilities when using our website, telemedicine platform, and in-office services (collectively, the “Services”).
By using or registering for our Services, you agree to these Terms and to our Privacy Notice and Notice of Privacy Practices. If you do not agree, please do not use the Services.
2. Services Provided
Lodo Pain provides low-dose pain management and related healthcare services through telemedicine (via doxy.me) and in-office appointments. Information on our website is for educational and scheduling purposes only and does not replace an evaluation by a licensed provider.
While we make every effort to provide accurate information, no medical relationship is created by viewing or using our website. A provider-patient relationship is established only when you are seen and accepted for treatment by a licensed Lodo Pain clinician.
3. Medical Disclaimer
The content on our website and any related communications (including emails, texts, or social media) is for informational purposes only. It does not constitute medical advice, diagnosis, or treatment.
If you are experiencing a medical emergency, call 911 immediately.
Lodo Pain does not recommend or endorse any specific tests, treatments, or products mentioned on our site. All care decisions are made individually by your provider.
4. Appointments and Telemedicine
Appointments are typically scheduled every 4 weeks, though timing may vary based on your treatment plan.
We offer both telemedicine and in-office visits:
-
Telemedicine visits are conducted via doxy.me, a secure and HIPAA-compliant platform.
-
In-office visits may be required based on your treatment plan, medication type, or applicable state and federal guidelines.
-
You must be physically located in Arizona at the time of a telemedicine visit, as our providers are licensed in Arizona.
You acknowledge that telemedicine has limitations and may not be appropriate for all conditions. Lodo Pain reserves the right to require in-person visits based on your treatment plan, provider judgment, or regulatory requirements.
5. Payment and Billing
Lodo Pain does not participate with any insurance carriers. All services are self-pay and are considered non-covered by insurance.
We accept credit and debit cards, and payment is due prior to the time of service.
If you need to cancel or reschedule, please provide at least 24 hours’ notice to avoid potential fees.
6. Your Responsibilities
By using our Services, you agree to:
-
Provide complete and accurate information during registration and treatment.
-
Use the Services only for lawful, personal healthcare purposes.
-
Maintain the confidentiality of your login information.
-
Follow your provider’s instructions and comply with your care plan.
Misuse of telemedicine, falsifying medical information, or using another person’s identity for care may result in termination of services and possible reporting as required by law.
7. Privacy and Data
Your personal and medical information is protected under federal and state privacy laws.
-
Non-medical website data is governed by our Privacy Notice.
-
Medical information is governed by our Notice of Privacy Practices (HIPAA).
By using our Services, you consent to the collection and use of your information as described in these notices.
8. Website Use
Our website is provided for informational and scheduling purposes only.
The content does not constitute medical advice and should not replace an evaluation by a licensed healthcare provider.

To the fullest extent allowed by law, Lodo Pain LLC, its providers, and affiliates are not liable for indirect, incidental, or consequential damages arising from your use of the Services.
If you are dissatisfied with the Services, your sole remedy is to discontinue use.
11. Dispute Resolution
