1. Policy and Scope
LoDo Pain, LLC (“LoDo Pain”) complies with applicable federal and Arizona civil-rights laws. LoDo Pain does not discriminate against, exclude, or treat individuals less favorably because of race, color, national origin—including limited English proficiency and primary language—age, disability, or sex, consistent with applicable law.
This policy applies to LoDo Pain’s healthcare services, programs, facilities, communications, and patient-access activities.
2. Equal Access to Healthcare Services
LoDo Pain provides healthcare services without unlawful discrimination. Eligibility for treatment and clinical decisions are based on appropriate factors such as:
- Clinical appropriateness;
- Patient safety;
- Available services and operational capacity;
- Provider availability;
- Insurance eligibility, network participation, referral requirements, and authorization requirements;
- Compliance with clinical and administrative policies; and
- Applicable federal and Arizona law.
Nothing in this policy guarantees acceptance into the practice, insurance coverage, authorization, a particular treatment, or the prescribing or continuation of any medication.
3. Assistance for Individuals With Disabilities
LoDo Pain provides reasonable modifications and appropriate auxiliary aids and services when necessary to give individuals with disabilities an equal opportunity to communicate effectively with us and access our services.
Available assistance may include:
- Qualified sign-language interpreters;
- Written information in accessible formats;
- Large-print materials;
- Accessible electronic documents;
- Assistance completing forms; and
- Other reasonable communication accommodations.
Appropriate aids and services are provided free of charge when required by applicable law.
4. Language Assistance
LoDo Pain provides appropriate language-assistance services to individuals whose primary language is not English when necessary to provide meaningful access to our healthcare services.
Language assistance may include:
- Qualified interpreters;
- Telephone or video interpretation;
- Qualified translation of written information; and
- Information provided in languages other than English.
- Required language-assistance services are provided free of charge.
LoDo Pain will not require a patient to provide or pay for their own interpreter when LoDo Pain is legally required to provide one. Except when permitted by law in limited circumstances, LoDo Pain will not rely on minor children, accompanying family members, friends, or unqualified individuals to interpret healthcare information.

Patients are encouraged to request assistance before an appointment when possible so appropriate arrangements can be made. Advance notice is helpful but is not required when the need for assistance could not reasonably have been anticipated.
6. Filing a Grievance With LoDo Pain
An individual who believes LoDo Pain failed to provide required language assistance, disability-related assistance, or a reasonable modification—or otherwise discriminated unlawfully—may submit a grievance to the LoDo Pain Civil Rights Contact.
A grievance may be submitted by mail, telephone, fax, or email using the contact information above. It should include, when available:
- The individual’s name and contact information;
- A description of what occurred;
- The date and location of the incident;
- The names of the individuals involved, if known; and
- The resolution being requested.
LoDo Pain will provide reasonable assistance to anyone who needs help submitting a grievance. The clinic will review the grievance promptly, conduct an appropriate investigation, and communicate the outcome in writing or another accessible format when appropriate.
Filing a grievance will not affect access to medically appropriate care. LoDo Pain prohibits retaliation against anyone who requests assistance, raises a discrimination concern, participates in an investigation, or files a grievance or civil-rights complaint.
7. Filing a Complaint With HHS
A civil-rights complaint may also be filed with the U.S. Department of Health and Human Services, Office for Civil Rights. A patient does not have to complete LoDo Pain’s internal grievance process before contacting the Office for Civil Rights.

8. Notice of Availability of Language Assistance Services and Auxiliary Aids and Services
LoDo Pain provides free language-assistance services to individuals whose primary language is not English when these services are necessary to provide meaningful access to our healthcare services.
LoDo Pain also provides appropriate auxiliary aids and services free of charge when necessary to ensure effective communication with individuals with disabilities. Available assistance may include qualified interpreters, information in accessible electronic formats, large-print materials, audio formats, and other appropriate communication assistance.

LoDo Pain will periodically review this policy and its language-access and accessibility practices. The clinic may revise this policy to reflect changes in applicable law, available services, organizational responsibilities, or regulatory guidance.
The current version will be posted on the LoDo Pain website and made available upon request.
